Elements of Transition Planning
Below are some examples of issues and tasks that are typically addressed as part of transition planning:
- Funding for Support Family services secured, including enrollment in Medicaid Waiver (if applicable)
- Pre-placement visits scheduled and coordinated
- Training for the Support Family specified, scheduled and completed
- Preparation for the child (individualized to child’s needs)
- Supports/Services identified, secured, and scheduled (as applicable)
- Medical: Nursing or other health assessments completed, physician identified and orders obtained, pharmacy identified, in-home nurses hired and trained.
- Equipment/Supplies: Needed equipment and supplies identified, Durable Medical Equipment (DME) company identified and contacted, delivery arranged on or before move date.
- Personal Care Services: Daily personal care needs/routine identified, attendant schedule determined as appropriate, staff hired/trained.
- Positive Behavior Supports: Assessments and recommendations obtained and plan developed, Support Family and other caregivers trained, in-home supports identified and included in care plan.
- Home Modifications: Needed home modifications identified and completed before move or scheduled after move as appropriate.
- Respite/Back-up Caregivers: Alternate caregivers identified and trained, schedule developed.
- Transportation: Need for specialized car seat, other adaptive aides, or vehicle modifications determined and equipment secured; transportation to family home on move day arranged; ongoing transportation secured.
- SSI/Medicaid –application for SSI, notification to Social Security Administration (SSA) of the change of address, payee designation
- School –discharge from old school/enrollment at new school, copy of ARD and other documentation sent to new school
- Moving Day – packing and transporting of all of the child’s belongings, medications and supplies to Support Family home; discharge of child from institutional placement; identification of support staff to be at the Support Family’s home to receive the child
Contacts –Written list of all important contact numbers for Support Family, child’s family/guardian, provider staff and others
